Join Canadian Stroke Research

This Canadian study led by the University Health Network is studying how exercise and medication might help reduce stroke related fatigue.

Participants may receive Modafinil or a placebo. Small studies suggest Modafinil may play a role in managing fatigue, but results are unclear.

This study compares exercise plus Modafinil with exercise plus a placebo. Exercise sessions will be delivered at home by a trained therapist.

What is required of participants?

  • Exercise will be completed over video call three times a week.
  • Fatigue, sleep, mood and activity levels will be measured.
  • Researchers will track changes for up to 6 months.

Who can Join?

  • Had a stroke confirmed by a doctor.
  • Have significant fatigue as the result of stroke.
  • Do not have advanced cancer, severe depression, pregnancy, or any other condition that would not allow for participation.

What does the study involve?

This study looks at whether medication and/or exercise can help in stroke recovery.

A stroke happens when blood flow to the brain is stopped and the brain gets damaged.

A lot of people report fatigue as one of the worst symptoms post stroke that can affect daily activities and the length and quality of life.

Post stroke fatigue (PSF) afflicts as many as 70% of the 400,000 Canadians with stroke. This type of fatigue does not improve with rest and can cause ongoing tiredness, exhaustion, and difficulty starting or keeping up with activities.

Research shows that post-stroke fatigue has many causes, including changes in the body, the muscles, the heart, and the brain.

A stroke can also affect sleep and mood, which can impact how people feel too.

There are currently no proven treatments, but both exercise and the medication Modafinil have shown early signs of promise in small studies.

This clinical trial tests the effect of exercise combined with Modafinil 200 mg daily or placebo medication over 8 weeks on the severity of fatigue symptoms. It aims to recruit 212 participants.

The main idea being tested is whether exercise plus Modafinil will improve fatigue more than exercise plus a placebo after 8 weeks.

Each person will be tested on fatigue, mood, fitness, thinking skills, sleep, and usual activity levels.

Participants will be assigned at random to 1 of 2 groups:

  1. Placebo  + Home-based Exercise
  2. Modafinil + Home-based Exercise

The researchers will assess the results to identify the best treatment for post stroke fatigue and hope to be able to find a treatment that will help reduce fatigue and improve quality of life after a stroke.
